    Bring a referral or prescription from your doctor or chiropractor detailing your injury and recommended treatment.

  • Claim Information
    Bring your claim number, insurance adjuster or case manager contact information, and any correspondence related to your auto accident.

  • Medical History & Current Condition
    Share any recent injuries, surgeries, or ongoing treatments with your massage therapist.

  • Comfortable Clothing
    Wear loose clothing or be prepared to undress to your comfort level for targeted areas.

  • Pain & Symptom Notes
    Note areas of discomfort or stiffness so the therapist can focus on the most affected muscles.

Work Injury Clients (L&I / Work Injury)
